Please attach important information and/or images needed for the consultation. Once we review the information, one of our staff will email you with a consultation date and time. As we are not your Primary Physician, we cannot prescribe any medications, only offer a second opinion and suggest procedures and products.

We offer both one half hour and one full hour consultations. Please remember to include your contact information, location and time zone, and amount of time you are requesting. Payment/fee below for the consultation is due prior to the appointment. After confirmation of your appointment by our Staff, Payment must be received 24 hours prior to Appointment or the Consultation is cancelled.
